Pattern Rotater's filetype was 'APPE' instead of 'appe'. If you don't want to spend the time downloading PatMan 1.5.1, you can simply change it's file type using ResEdit or any of the other file-type changers out there. Pattern Manager--PatMan--displays patterns with their names in a user-sizable grid, installs patterns into the System File, and sets the utilities pattern (which other desktop pattern installers don't do).
